What is SMS Verification in Forms and Surveys?
SMS Verification adds an extra verification step to the Phone field in a Form or Survey. It helps confirm that the person completing the Form or Survey can receive messages at the phone number they entered, which can improve the reliability of the contact information you collect.
When SMS verification is enabled, the respondent enters their phone number and selects Verify Phone. System sends an OTP to that number, and the respondent enters the code to complete verification.
The verification status can also be reviewed in the Form or Survey submission data using the Phone Verified field.

Enabling SMS verification directly on the Phone field ensures respondents can verify their number before submitting their information.
Note: To enable SMS OTP Verification, please ensure your phone number is setup in your account settings.
Go to Sites in the left navigation menu.
Select Forms or Surveys.
Create a new Form or Survey, or open an existing one in the builder.

Add a Phone field to the Form or Survey if one has not already been added.
Select the Phone field to open its field settings.
Scroll through the Phone field settings and enable Validate Phone.
Preview, Save and integrate the Form or Survey.
